If you need to fax forms, please send them to 309-857-0157. Vaccination, Dental, Physical, and Vision requirements can be found under Annual Immunization Requirements and Data Summary for the annual compliance date of October 15.

When students or staff do not feel well, they should stay home and follow the appropriate protocols.   Absences should be reported as expected in the past.

• Fever - May return when fever free for 24 hours without use of fever reducing medication.

• Vomiting - May return when vomit free for 24 hours without use of anti-emetic medication.

• Diarrhea - May return when diarrhea free for 24 hours without use of anti-diarrheal medication.
